Ontario Health Teams Now Providing Care In Every Corner of the Province

The province is marking an important milestone in achieving full provincial coverage of Ontario Health Teams with the approval of the final team in West Parry Sound, bringing support to Ontarians in every corner of the province and ensuring faster and easier access to care.

Working together, these 58 Ontario Health Teams transform the way people connect to care by ensuring a seamless transition from one provider to another, with one patient record and one care plan being shared between their providers.

Starting in April 2024, the government is investing up to $140.5 million to provide every Ontario Health Team with $2.25 million over three years. This funding will better coordinate people’s care journey by connecting them to different types of care and providing 24/7 help navigating the health system.

As the government implements actions through Your Health: A Plan for Connect and Convenient Care, the government is ensuring that every person in Ontario can connect to the supports and high-quality care they need, where and when they need it.
